Do you have a fever of 100 or have you had a fever of 100 in the last 14 days? 3. Are you experiencing cough, sore throat, chills, headache, muscle pain, new loss of taste or smell or shortness of breath? (two or more symptoms) 4. Have you been exposed to someone who has been diagnosed with COVID-19 or is experiencing these symptoms? 5. Have you been out of the country in the past two months?
